Here are a list of forms for member's and club official's use. We are trying to get most of our forms online, so that they are available for you at any time, and very quickly.

Membership Forms 

 Club Membership Forms - For use by a club to provide officer information, individual/family memberships, and business memberships to Snowmobile North Dakota.  (Excel Format)

*If the computer being used to access these forms does not have Excel or PDF software, these forms will produce an error message.
*Snowmobile North Dakota does not offer online membership capabilities.  The Excel membership forms are fillable, however, the information must be printed and mailed for processing. 

Trail Forms

2010 ND Snowmobile Trail Application - This application is due April 1st, 2010 and is required for any trail associations/clubs requesting funding, equipment, or trail changes.  The following documents are samples of completed trail applications.

Land Lease Form -  Please complete this form for land use easements.
         Recreational Immunity Act (PDF) - Information on liability for 
          property owners allowing recreational use on their personal 
          property.

Event Notification & Enforcement Request Form (PDF) - Please complete the form in order to provide Snowmobile North Dakota with a list of upcoming events and to request trail enforcement and safety education classes at the events.

Volunteer Contribution Report (PDF) - This form is to be used by volunteers to report volunteer hours!
Volunteer Contribution Report (Excel) - For those of you who have Excel on your computers, this form will automatically total all of your volunteer hours.

"Of the year" nomination forms - Dealer- Snowmobiler- and Snowmobiling Family of the Year nomination forms.

SND Hall of Fame - Instructions on nominating a remarkable individual (s) that has helped North Dakota's snowmobile industry.

Commendation/Complaint Form - Please take the time to complete this form and provide your commendations or complaints regarding North Dakota's snowmobile trails. 

Membership Card Discount Report - Please complete this form if you are willing to participate in the Membership Card Discount Program Snowmobile North Dakota offers.  You may fax (701-328-5363) or mail the report to Snowmobile North Dakota.
